Aftercare Services
Your clinical team collaborates with you to create an aftercare plan designed to facilitate your transition from treatment to independence.
An effective plan paves the way for your success and empowers you to continue your recovery on your own terms.
We provide a variety of programs for men and women, including outpatient services, Partial Hospitalization Programs (PHP), Intensive Outpatient Programs (IOP), and a 28-day inpatient program for men. Our continuum of care also includes graduate housing options. Our dedicated team is committed to offering support at every stage of the recovery journey.
At Restored Life, we believe that the conclusion of your official treatment marks the beginning of your new journey—and you won’t have to navigate it alone. As a member of our alumni network, you’ll receive a comprehensive discharge folder that contains your medical records, clinical insights gathered by your primary therapist, and the essential document that can support your ongoing sobriety.
Aftercare Plan incorporates these crucial elements:
Your Aftercare Plan will detail the methods to achieve key goals, including exercise locations, hobbies, stress management strategies, living arrangements, peer support meeting sites, and medical care providers. You'll receive a comprehensive list of names, dates, addresses, appointment times, and resources to maintain your progress.
At Restored Life, we understand that the longer you remain in treatment, the lower your chances of relapse. Aftercare is not just a phase—it's an essential continuation of your treatment as you re-enter the world. While it may be a different kind of experience, it demands your commitment, discipline, and diligence. Success in Aftercare calls for self-management, self-awareness, and radical honesty. It requires humility and effort—but you don’t have to embark on this journey alone.
In fact, successful Aftercare thrives on connection. Essential elements such as peer support, family involvement, and engaging in hobbies all hinge on the positive contributions from others. Reflect on the old saying, "It takes a village to raise a child,” and consider how this wisdom applies to your path to sobriety. The role of community is pivotal in achieving long-term recovery.
At Restored Life, our Alumni Network is dedicated to providing that crucial community support. We’re here to help you discover a safe and nurturing environment wherever you choose to settle after your time with us. Together, we can facilitate your journey towards sustained sobriety and a fulfilling life.
Continuum of Care
In the first phase of treatment, clients receive 24/7 comprehensive care at the facility. Our team of medical professionals, therapists, and case managers provides structured support throughout the day. Clients participate in 21 hours of therapy each week, and our support staff attends to all daily needs. Many of our staff members are also in recovery, offering empathy and understanding at every step of the journey.
In the second phase, known as PHP, individuals with moderate Substance Use Disorder (SUD) participate in five hours of therapy sessions, five days a week, while maintaining family life and obligations. The program typically lasts 3-4 weeks and involves coordinated care from psychiatrists, nurse practitioners, therapists, and case managers.
As a further step-down in the continuum of care, outpatient treatment involves group therapy sessions for three hours a day, three days a week, conducted by credentialed therapists. This phase allows clients more time for family and potential reintegration into work life.
Our aftercare program is for clients who have completed the Restored Life Recovery program. Alumni receive guidance and support from a dedicated provider and participate in regular group gatherings to discuss issues, share successes, and foster a supportive community beyond formal treatment.
